How Do I Start Therapy?
If you are interested in scheduling an evaluation for your child, call our office with your health insurance information. After a quick phone consultation, we will call your health insurance carrier and determine your benefits.
We are currently an in-network provider with the following:
- BlueCross BlueShield
- Health Partners
- Medica
- United Health Care
- Preferred One
- Minnesota Care
If necessary you can access our services using your out-of-network benefits. We’ll take care of all insurance submissions for you, as well as any authorizations or referrals that you may need. Once you have scheduled an appointment, please download the new patient information forms and bring the filled out forms with you to your child's first appointment.
What do I need to bring to my child's first appointment?
- New patient info forms
- Health Insurance Card
- Socks for you and your child. Shoes are not permitted in our treatment areas.

Individual Forms
If you are just looking for a specific form, all forms are provided individually here.
- Evaluation Sheet - Provides important information to complete a thorough evaluation of your child. Depending on your child’s abilities, some questions may not be applicable.
- Patient Info Form - Authorizes the Madison center to release and/or obtain information from listed physician.
- Insurance Form - Authorizes your insurance to make payments direct to The Madison Center.
- Confidentiality Form - Authorizes The Madison Center to release or obtain information for the purposes of collaboration of services.
